LVN Intake Liaison Coordinator

Locations: La Mesa, CA

Currently seeking LVN Intake Liaison’s who are interested in working as Independent Contractors. RN’s are also encouraged to apply. The role is primarily remote, but will require some onsite as needed.

Responsibilities of LVN Intake Liaison Coordinator:

  • Obtains documents for services being requested.
  • Has the ability to demonstrate compassion and courtesy when dealing with difficult
  • situations.
  • Capable of working under pressure and time constraints.
  • Ability to use sound judgment and tactful approach when dealing with co-workers, staff,
  • and the public.
  • Must possess the ability to use and apply clinical knowledge to the referral process.
  • The Intake Coordinator has direct contact with referring physicians, hospital liaisons and patient/families. The Intake Coordinator organizes and coordinates daily intake/referral processes for the CLHF to ensure that the initiation of care is timely and according to physician orders and within regulatory guidelines.
  • Acts as the key linkage between hospital case management services, medical group physicians and office staff.
  • Responsible for the coordination of complex patient issues related to CLHF services.

Job Requirements

  • Must be able to respond immediately to patient referrals that come in.
  • Sales and negotiating experience via telephone preferred.
  • Willingness to take direction and implement established processes.
  • Ability to overcome obstacles and problem-solve to effectively schedule sign-on appointments.
  • Self-motivated and able to work professionally in a fast-paced, ever-changing
  • environment.

Job Type: Per Diem, Remote

Required Education: Graduate from an accredited school of professional nursing
Required license or certification:

  • Current California LVN License
  • Current BLS Certificate
