Non-profit Organization is recruiting a Nurse Care Manager (NCM) for the Multipurpose Senior Services Program (MSSP) and other related programs. Non-profit Organization has worked to improve the health of people the Central Coast since 1980. We are looking for a caring and creative individual to make a difference in the lives of people in need. We pride ourselves on featuring an innovative, friendly and flexible work environment.


The NCM is responsible for providing care management to frail, low income, elderly clients. The NCM will evaluate the client and identify the functional and health limitations that impede independent living. The NCM responsibilities include the following activities:

  • Conducting thorough in-home assessments covering medical, health, and rehabilitation concerns;
  • certifying level of care determinations;
  • performing physical assessments and interpreting clinical findings;
  • providing medication management;
  • working closely with physicians and other health professionals;
  • participation in developing care plans, implementing services, conducting follow up and monitoring of client’s needs and care plan;
  • identifying and developing support systems for the client;
  • electronic case recording and reporting.


Requirements include:

  • A valid current license (RN) to practice nursing in the State of California;
  • Minimum of one-year clinical experience;
  • Bilingual English/Spanish preferred;
  • Experience performing geriatric assessments and care preferred;
  • A positive and healthy attitude toward clients, the agency and its staff and collaborating programs;
  • Ability to travel within Monterey and Santa Cruz County regularly to meet clients in their homes;
  • Current CA driver’s license and a reliable, insured vehicle.
